At Honoring Choices® Delaware, we partner with community groups, clinicians, and leaders in health and government to ensure you receive superior, patient-centered care throughout your life. Part of this care includes documenting your goals, values and preferences in an Advance Directive – a written plan your loved ones and health care providers can follow in the event you can’t speak for yourself.
